We are looking for a Nurse Practitioner (NP) with experience in patient-centered, trauma-informed, and recovery-based care for the AMH Outreach Team in Lethbridge, Alberta. This position is based out of the Lethbridge Provincial Building and will provide concurrent support to individuals who require treatment in place for a variety of medical conditions including chronic disease, acute wound care and STBBI testing/treatment. Services will be community based and recovery oriented towards adults experiencing severe and persistent mental illness, addiction, and homelessness. You will work in collaboration with community partner teams, both AHS and non-AHS based to collaboratively provide care to individuals whose lives have been impacted by unstable housing, socioeconomic factors, and substances. The primary focus of the Nurse Practitioner (NP) role is diagnosing and managing common and complex health conditions, promoting health, and preventing illness/injury, ordering and interpreting diagnostic tests, prescribing pharmaceuticals, other therapies, and performing procedures, interventions and/or treatments.
This position requires a Nurse Practitioner to assess and treat clients in the community. This may include assessing/testing and treating clients for sexually transmitted and blood borne infections (STBBI), prescribing non- pharmacologic and pharmacologic treatment options such as opioid replacement therapy and primary care services. The Nurse Practitioner must be authorized by the College and Association of Registered Nurses of Alberta to prescribe controlled drugs and substances including Suboxone. The successful candidate will foster collaborative partnerships with clients, physicians, community-based services, and agencies to enhance the provision of services to clients and families, in alignment with the AHS endorsed harm reduction strategies. Collaborates and participates on working groups and committees to further enhance and develop the zone AMH programs and services. Collaborates with AMH staff, Public Health staff and physicians and other care providers to assess and treat clients living with or newly diagnosed HIV, HCV and support for detoxification clients with complex addiction, mental Health, and medical conditions.
Successful completion of an approved NP program at a minimum of a Master of Nursing level. Registration with CARNA as an NP.
Knowledge and/or experience of addiction and/or mental health treatment modalities (e.g., motivational interviewing, cognitive behavioral therapy, and solution focused therapy) is an asset. Experience providing Addiction and Mental Health/Public Health services is preferred. Experience engaging complex clients in a variety of settings is an asset. Knowledge and/or experience of harm reduction strategies is an asset. Knowledge of testing and treatment pathways for HIV and HCV.
