Nurse Pracitioner

Nurse Pracitioner

Full time | Vancouver, BC

Description

Value-based care is a concept that refers to ensuring that we at Providence Health Care are prioritizing the care and resources that matter most to the people we serve.

Our Nurse Practitioners are responsible and accountable for the comprehensive assessment of patients, clients and residents, including diagnosing diseases, disorders, and conditions.

We are seeking passionate health care professionals to come and join our team of Nurse Practitioners. Currently we have opportunities within our Geriatric Psychiatry, Older Adult Medicine, and Foundry departments.

About our Geriatric Psychiatry program:
“One South” is the 16 bed Geriatric Psychiatry Unit (GPU) for older adults with psychiatric illness such as depression, schizophrenia, and bipolar disorder or dementia with psychosocial and behavioural symptoms. “Four East” is a 22 bed Geriatric Medicine Reactivation Unit that provides sub-acute medical care to older adults. Four beds are designated as shared care between Geriatric Medicine and Geriatric Psychiatry providing care to patients with psychiatric illness and medical concerns.

Expected length of stay for our patients is between two to six weeks. During their stay, patients will be assessed by psychiatrists, family doctors, geriatricians, occupational therapists, physiotherapists, dieticians, pharmacists, social workers and nurses. A treatment plan including medications and non medication treatment such as behavioural approaches will be formulated prior to discharge. The goal of treatment is to assess, treat and stabilize patients before discharging them back home.

About our Older Adult Medicine program:
About 25 per cent of all patients admitted to St. Paul’s Hospital and about 50 per cent of patients admitted to Mount Saint Joseph Hospital are over the age of 70. The Elder Care Program’s multidisciplinary teams assist these elders with their acute health care needs, including providing care to those who have experienced significant functional decline within the last two to four weeks, or who are experiencing multiple complex medical and/or psychiatric problems that need to be addressed. In some situations, these elders may require rehabilitation/reactivation to return home. Early access to geriatric expertise is fundamental to avoiding or minimizing the loss of functional abilities in frail elders. It provides better care, a shorter recovery and shorter lengths of hospital stay.
