Primary Care Nurse Practitioner/Clinical Supervisor | Full Time | Trenton/Quinte West

Are you a Nurse Practitioner who thrives in a leadership role and is passionate about delivering high-quality, community-based care? We are seeking an Nurse Practitioner and Clinical Supervisor to co-lead our community clinic, providing acute and episodic care to unattached patients. You will have the unique opportunity to oversee health programs, provide direct care, and manage a dedicated team of healthcare professionals, ensuring our clinic’s services align with our mission to support underserved populations in Brighton and Quinte West.
This role offers both hands-on clinical duties and the chance to shape our healthcare programs through close collaboration with our Executive Director. You’ll make a meaningful impact on your community while working in a supportive, dynamic, and team-oriented environment
.Key Responsibilities
Clinical Leadership: Provide patient-centred care, including assessments, diagnoses, treatments, and health education. Deliver direct care and support chronic disease management, preventive care, and health promotion services. Collaborate closely with physicians and maintain accurate and timely patient records.
Supervisory Leadership: Oversee Nurse Practitioners, Registered Nurses, and Registered Practical Nurses. Foster a positive, team-oriented culture by leading internal communication, setting performance goals, and supporting training and professional development. Ensure compliance with healthcare regulations and legislation.
Program Development: Collaborate with the Executive Director to design, implement, and evaluate innovative health programs and care delivery models. Provide input on long-term strategic planning for clinical services.
Operational Support:
Manage patient appointment scheduling, oversee referrals, and ensure accurate documentation. Contribute to the continuous improvement of clinic workflows and patient care standards.
Requirements
Education: Current registration with the College of Nurses of Ontario as an RN (extended class). NP-Primary Health Care specialty certificate required. A Master’s degree and diabetic educator certification are assets.
Experience: Proven track record in primary care or community health settings, especially in serving populations with barriers to care. Experience leading teams in a clinical setting is highly preferred.
Skills: Strong leadership, communication, and problem-solving skills. Ability to work independently while fostering a collaborative, team-oriented culture. Comfortable in a fast-paced environment with an integrated care model. Knowledge of community health resources is a plus.
