Nurse Practitioner –Permanent Full-Time Position, Quest Community Health Centre
Quest CHC is a dynamic non-profit, interdisciplinary health care organization. Our holistic model of care
focuses on primary care, illness prevention, health promotion, community capacity building and service
integration.
Quest CHC delivers healthcare that addresses the underlying social determinants of health, such as
poverty, housing, unemployment and food security, all which have a significant impact on health

QUEST CHC OFFERS:
Salary Range: $100,186.03 to $124,187.72 annually, ($51.38 to $63.69 per hour)
Plus: HOOPP (Pension Plan), Benefits, Vacation, Personal Leave Days, and $1,000 annual training funding
Program Area: Primary Care
Employment Status: Full Time/Part Time also available
Hour: 37.5 per week
Requirements
The Nurse Practitioner is responsible for the provision of primary health care services in
accordance with the Standards of Practice for Registered Nurses in the Expanded Class
and the Expanded Services for Patients Act, and other clinical activities as set forth by the
College of Nurses of Ontario.
The NP is responsible for ongoing health care, has a strong emphasis on health promotion,
disease prevention and community capacity building, and works within a harm reduction and
trauma-informed framework
The ideal candidate will maximize connections and use of existing community resources by
acting as a catalyst who, together with community stakeholders, will facilitate the
development and implementation of creative and efficient strategies to maximize health
outcomes for our clients, while maximizing the use of community care plan.
